Part-Time Social Worker

About the Role

The Social Worker facilitates patient care continuity through resource coordination, assessment, and intervention to promote safe discharge, prevent readmissions, and support patient self-determination.

Responsibilities

• Conducts complex psycho-social assessments
• Develops and reassesses transition plans
• Assists with cases involving adoption, abuse, and neglect
• Leads multidisciplinary care conferences
• Communicates care plans to patients and families
• Ensures compliance with regulatory standards
• Provides education to stakeholders
• Manages transition planning and coordinates care implementation
• Leads case reviews
• Serves as a resource within the care team to enhance patient outcomes and satisfaction

Requirements

• Bachelor's of Social Work
• Current licensing or license eligibility as LICSW, LCSW, or LMHC
• Preferred two years of acute hospital experience
