Part-Time Social Worker

About the role

The Social Worker facilitates patient care through effective resource coordination to promote optimal health, access to care, and appropriate resource utilization.

Responsibilities

• Conduct psycho-social assessments and interventions.
• Support transition planning for patients at risk of readmission.
• Lead multi-disciplinary care conferences.
• Assist with adoptions, abuse, and neglect cases.
• Communicate plans of care with patients and families.

Qualifications

• Bachelor's of Social Work.
• Current licensure or eligibility as a LICSW, LCSW, or LMHC.

Preferred Experience

• At least 2 years in an acute hospital setting.
