Part-Time Social Worker

About the role

Provide psychosocial support and social work services to hospice patients and their families in various settings, including inpatient, nursing home, and home care.

Responsibilities

• Develop and implement social work care plans, set goals, and make recommendations based on patient and family needs.
• Offer pre- and post-bereavement services.
• Serve as a liaison between hospice, inpatient, and nursing home staff, and participate in discharge planning when applicable.
• Maintain accurate documentation of assessments and interventions.
• Collaborate as part of an interdisciplinary team to support patient care.

Requirements

• Graduate from an accredited School of Social Work.
• Minimum one year of healthcare social work experience.
• NY State MSW licensure required.
• Proficiency in Word and Excel preferred.
• Excellent organizational, communication, and listening skills.
• Ability to input clinical data proficiently.
• Valid driver’s license and access to a vehicle.
