Part-Time Social Worker

Role Overview: Provide psychosocial support and social work services to hospice patients and families across various settings, including inpatient units, nursing homes, and home care. Develop and implement social work care plans, set goals, and make tailored recommendations based on patient and family needs. Key Responsibilities Offer pre- and post-bereavement services to patients and families.Serve as a liaison between hospice, inpatient, and nursing home staff, participating as a member of the interdisciplinary team.Maintain accurate documentation of assessments and interventions.Assist with discharge planning for inpatient admissions when necessary. Qualifications & Skills Graduate from an accredited School of Social Work.Minimum of one year healthcare social work experience.Knowledge of Word and Excel preferred.Valid NYS MSW license required.Ability to work collaboratively within an interdisciplinary team.Excellent organizational, communication, and listening skills.Proficient in clinical data entry.
