Part-Time Social Worker

Role: Provide psychosocial support and social work services to hospice patients and their families across various settings, including inpatient units, nursing homes, and home care. Key Responsibilities Deliver pre- and post-bereavement services to patients and families.Serve as a social work team member and liaison among hospice, inpatient, and nursing home staff.Develop social work care plans, set goals, and make tailored recommendations based on assessed needs.Maintain accurate documentation of assessments and interventions.Participate in discharge planning for inpatient admissions. Qualifications & Skills Graduate of an accredited School of Social Work.At least one year of healthcare social work experience.Knowledge of Word and Excel preferred.Current NYS MSW licensure required.Valid NYS driver’s license preferred.Strong organizational, communication, and listening skills.Ability to input clinical data proficiently.
