Part-Time Social Worker

Role Overview: Provide psychosocial support and social work services to hospice patients and their families in various settings, including inpatient units, nursing homes, and home care. Key Responsibilities Develop and implement social work plans of care, set goals, and make recommendations based on patient and family needs.Offer pre- and post-bereavement services to support families through grief.Serve as a liaison between hospice, inpatient, and nursing home staff, and be a member of the interdisciplinary team.Maintain accurate documentation of assessments and interventions.Participate in discharge planning for inpatient admissions. Qualifications & Skills Graduate from an accredited School of Social Work.Minimum of one year healthcare social work experience.Knowledge of Word and Excel preferred.Licensed as an MSW in New York State.Excellent organizational, communication, and listening skills.Ability to work within an interdisciplinary team and input clinical data proficiently.
