Part-Time RN Home Health

About the role

The RN Home Health provides comprehensive patient assessments, develops and coordinates individualized care plans, and performs routine to complex nursing procedures in the patient's home.

Responsibilities

• Conducts initial, recertification, and routine visits
• Collaborates with physicians, patients, families, and team members to ensure appropriate care
• Educates patients and caregivers to promote self-care and recovery
• Communicates patient status and lab results
• Ensures compliance with regulatory guidelines
• Participates in performance improvement activities
• Maintains documentation accuracy and timeliness
• Supports staff education and professional development
• Adheres to organizational policies

Qualifications & Skills

• Licensed as a registered nurse in Georgia with at least 1 year of Medical/Surgical or Home Health experience
• Strong critical care nursing skills
• Proficiency in computer data entry, communication, and basic software (Excel, Word)
• Certifications in BLS and valid Georgia Driver's License
• Ability to work collaboratively in a team environment and participate in quality improvement initiatives
