Part-Time RN Home Health

Role: The RN Home Health provides comprehensive patient assessments, develops and coordinates individualized care plans, and performs routine to complex nursing procedures in the patient's home. Responsibilities: Conducts initial, recertification, and routine visits; collaborates with physicians, patients, families, and team members to ensure appropriate care; educates patients and caregivers to promote self-care and recovery; communicates patient status and lab results; ensures compliance with regulatory guidelines; participates in performance improvement activities; maintains documentation accuracy and timeliness; supports staff education and professional development; and adheres to organizational policies.Qualifications & Skills: Licensed as a registered nurse in Georgia with at least 1 year of Medical/Surgical or Home Health experience; strong critical care nursing skills; proficiency in computer data entry, communication, and basic software (Excel, Word); certifications in BLS and valid Georgia Driver’s License; ability to work collaboratively in a team environment and participate in quality improvement initiatives.
