Responsibilities
- Perform visits and complete coordination of client care
- Serve as the point of contact for all disciplines involved with providing care to patients and oversee the frequency of visits for the episode
- Consult as needed with the physician and the office regarding patient care details
- Perform accurate OASIS collection
- Ensure the medication profile remains current
- Ensure lab values are reported to the physician in a timely manner
- Attend weekly case conference and monthly case manager meetings
This part-time salaried position works 24 hours per week, over 4 days in the week, and is eligible for our 60% paid days off program.
Qualifications
- Currently licensed as a Registered Nurse (RN) in the state of employment
- A minimum of one year of clinical experience is preferred
- Demonstrate knowledge and skill in current nursing practice
- Possess a valid state driver’s license and automobile liability insurance
