Core Job Responsibilities
- Captures and manages program data and supports the development and maintenance of reporting tools for submission to internal and external venues.
- Coordinates the development, design, implementation, and day-to-day operations of male involvement programming at Westside Healthy Start sites.
- Engages assigned or referred father/male participants in Westside Healthy Start male involvement case management curriculum across all Westside Healthy Start sites, including, assessing participant needs, developing service plans, and connecting participants to appropriate community resources.
- Implements and facilitates male involvement group education sessions in accordance with grant requirements.
- Provides follow-up support, including call-backs and ongoing engagement to promote sustained participation in male involvement activities.
- Establishes and maintains external partnerships to support referrals and resource connections for program participants (e.g., employment opportunities, community services).
- Develops and coordinates engagement activities that promote father involvement, co-parenting and child development activities.
- Prepares and distributes monthly reports summarizing program activities, participation and outcomes in accordance with grant and leadership requirements.
- Collaborates with Westside Healthy Start Community Health Specialists at each Westside Health Start site to ensure co-parenting activities are successful.
Requirements/Preferences
- Bachelor’s degree in a health-related field (social work, psychology, counseling, rehabilitation, sociology, or other human service field) preferred.
- Minimum 2 years of working in a community based or public health setting.
- Minimum 2 years of case management experience in a public or private social service program or health care setting.
- Intermediate proficiency with Microsoft Office products and database applications (i.e. Redcap, Electronic medical record).
- 2 years of project management experience in working with high-risk populations preferred.
