About the role

We are seeking a dedicated Registered Nurse (RN) to provide home health care in a part-time capacity. The role involves serving as the primary clinical partner for patients and families, delivering personalized nursing care in patients' homes to promote recovery, safety, and independence.

Responsibilities

• Conduct comprehensive in-home assessments.
• Provide skilled interventions such as wound care, infusions, medication management, and post-operative care.
• Educate and empower patients and caregivers.
• Monitor patient progress and communicate with the care team.
• Ensure accurate documentation to support quality and compliance.

Requirements

• Active RN license.
• At least one year of clinical experience.
• Valid driver’s license.
• CPR certification.
