Part Time PRN Home Health Registered Nurse

About the Role

Provide personalized, in-home nursing care to patients, focusing on recovery, safety, and independence. Conduct assessments, develop care plans, and deliver skilled interventions such as wound care, infusions, medication management, and chronic disease support. Educate patients and caregivers, monitor progress, and coordinate care with healthcare teams to ensure quality outcomes. Maintain accurate documentation to support compliance and patient progress.

Responsibilities

• Serve as primary clinical partner for patients and families
• Perform comprehensive in-home assessments
• Deliver skilled nursing interventions tailored to patient needs
• Educate and empower patients and caregivers
• Monitor patient progress and communicate with care teams
• Coordinate care across providers and services
• Complete timely, accurate clinical documentation

Requirements

• Active RN license in practicing state
• At least one year of clinical nursing experience
• Patient-focused, organized, motivated to deliver high-quality care
• Valid driver’s license and reliable vehicle
• CPR certification (or ability to obtain)
