Home Health Registered Nurse PRN Part Time

Role Overview

This position involves providing home health nursing care, serving as the primary clinical partner for patients and families.

Responsibilities

• Deliver individualized, one-on-one nursing care in patients' homes.
• Assess physical, emotional, and environmental needs to create care plans.
• Perform skilled nursing procedures, including wound care, infusions, and chronic disease management.
• Educate and empower patients and caregivers for symptom and medication management.
• Monitor patient progress, communicate with the care team, and adjust plans as needed.
• Ensure documentation is accurate and timely to support quality and compliance.

Qualifications & Skills

• Active RN license with at least one year of clinical experience.
• Patient-focused, organized, and motivated by delivering high-quality care.
• Valid driver’s license, reliable transportation, and CPR certification (or ability to obtain).
