Compensation & Scheduling

  • Part-Time: Up to 24 visits per week
  • Competitive Pay: $70.00 - $85.00 per routine visit

Key Responsibilities

  • Develop and implement individualized treatment plans
  • Conduct assessments using standardized tools and clinical judgment
  • Provide therapy focused on mobility, strength, balance, and safety
  • Educate patients and caregivers on home exercise programs and adaptive strategies
  • Maintain accurate and timely clinical documentation
  • Collaborate with a multidisciplinary care team to ensure coordinated patient care

Requirements

  • Licensed Physical Therapist in Colorado
  • Experience in home health or a related field
  • Strong communication and interpersonal skills
  • Proficient in electronic medical record documentation
  • Able to develop and implement individualized treatment plans
