Responsibilities
- Evaluate residents and develop individualized treatment plans.
- Provide therapy focused on Activities of Daily Living (ADLs), mobility, safety, and independence.
- Monitor progress and adjust treatment plans as needed.
- Educate residents and caregivers on exercises, adaptive techniques, and equipment.
- Complete accurate Net Health documentation and collaborate with the care team.
Qualifications
- Active and current Wisconsin Occupational Therapist license.
- Graduate of an accredited Occupational Therapy program.
- Current CPR certification.
- Reliable transportation and the ability to travel between both facilities.
- Strong communication, organization, and documentation skills.
- Geriatric, assisted living, skilled nursing, or rehabilitation experience is preferred.
