About the role
Licensed Occupational Therapists to provide personalized, in-home and senior living care to older adults, enhancing mobility, independence, and quality of life through our Geriatric House Calls™ model.
Responsibilities
- Conduct evaluations and create individualized care plans
- Deliver evidence-based therapy
- Document in EMR
- Collaborate with a multidisciplinary team
- Educate patients, caregivers, and staff to promote safety and aging in place
- Incorporate telehealth where appropriate
Qualifications
- Valid Occupational Therapy license
- Degree from an accredited Occupational Therapy program
- Computer proficiency
- CPR certification
Physical demands
- Assisting with patient transfers
- Repositioning
- Driving to care locations
