What You'll Do

• Perform nutrition assessments, education, and personalized care planning.
• Provide nutrition education and support for patients with complex medical conditions.
• Educate and guide patients on nutrition-related health management.
• Collaborate with a multidisciplinary healthcare team including RD, Speech, Occupational, and Nursing to deliver high-quality, patient-centered care.
