Who We Are Looking For

We are currently seeking an Advanced Practice Provider (APP) with an FNP, NP, or Adult Gero NP certification, or a PA, based in Wyoming, MI. This role serves local skilled nursing facilities within a mission-driven care community.

What You Will Do

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues.
• Communicate with facility nursing staff via phone or secure messaging as needed.
• Maintain reliable on-site attendance for scheduled shifts.
• Ensure compliance with clinical standards, facility policies, and regulatory requirements.

Qualifications

• Active U.S. license and board certification by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants).
• Qualified specialties include but are not limited to Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
