About the role

Part-time Clinician (Nurse Practitioner or Physician Assistant) providing in-home health evaluations to improve patient outcomes and bridge care gaps.

Responsibilities

• Conduct 45-minute home visits to review medication and medical history, perform physical evaluations, and administer diagnostic tests as ordered.
• Utilize an iPad for clinical workflow, offer personalized care, and conduct virtual visits when needed.
• Build trusted relationships with patients in diverse home environments to support their health and connect them to appropriate care.
• Provide comprehensive, personalized health assessments in patients' homes.
• Engage with patients to address health questions and review medical histories.
• Conduct diagnostic tests such as spirometry and diabetic eye exams.
• Collaborate within a national network of clinicians dedicated to improving health outcomes.
• Maintain safety and quality standards in diverse home environments.
• Travel locally and perform virtual visits as needed.

Requirements

• Active, unrestricted licenses (Nurse Practitioner or Physician Assistant).
• Board certification.
• Multi-state licenses preferred.
• Ability to work across specified regions.
• Focus on safety and comfort in varied home settings.
• Flexibility to travel locally.
