Part Time - Nurse Practitioner or Physician Assistant

About the role

A Part-Time Clinician (Nurse Practitioner or Physician Assistant) providing in-home health evaluations to enhance patient care and bridge gaps in health services.

Responsibilities

• Conduct 45-minute in-home visits.
• Review medication and medical history.
• Perform physical evaluations.
• Administer diagnostic tests as ordered.
• Utilize an iPad with an intuitive workflow; no prescribing or altering treatments.
• Conduct virtual visits as needed.
• Provide diagnostic services such as spirometry and screenings.
• Build trusted relationships with patients to improve health outcomes.
• Visit diverse home environments, ensuring safety and comfort.
• Travel locally and within licensed states, providing culturally competent care.
• Foster connections to appropriate health services based on individual needs.

Qualifications

• Active, unrestricted license(s) and board certification.
• Preferred multi-state licensing.
• Ability to work in primary locations such as Saint Cloud and surrounding areas.
• Comfortable working in diverse home environments, including homes with pets or allergens.
