Part Time NP or PA, Patient Connect, Senior Community Care - King County WA

Optum Home & Community Care, part of the UnitedHealth Group family of businesses, is creating a new integrated care model that holistically addresses individuals' physical, mental, and social needs. This role is within the Senior Community Care (SCC) product, focusing on providing care to patients at home, in nursing homes, assisted living, or senior housing. The SNF Patient Connect program aims to reduce 30-day hospital readmissions, improve diagnosis coding accuracy, close STAR/HEDIS quality measures, enhance completion of Advanced Care Planning Directives, and boost patient and family satisfaction.

About the Role

The Optum practitioner will conduct one-time comprehensive health assessments for members in collaboration with the case management team to support better care coordination and health outcomes. This autonomous role offers significant impact and satisfaction as you improve care for our population.

Responsibilities

• Work with primary care physicians to provide the best care possible
• Collaborate with nursing staff and patients' families
• Collaborate with the case management team
• Documentation, coding, and gap closure

Requirements

• Active and unrestricted Nurse Practitioner or Physician Assistant license in the state of residence
• Certified Nurse Practitioner through a national board (AANP or ANCC) or Physician Assistant through an accredited program
• For NPs: Graduate of an accredited master's degree in Nursing (MSN) program; board certification as ANP, FNP, or GNP (preferred), or Adult-Gerontology Acute Care Nurse Practitioners (AG AC NP)
• Current active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
• Driver's license and access to reliable transportation to travel to SNF facilities within a designated area
• Ability to move a 30-pound bag in and out of a car and navigate stairs and various dwelling conditions
• Ability to gain a collaborative practice agreement, if applicable in your state

Preferred Qualifications

• 1+ years of hands-on post-grad experience within Long Term Care
• Understanding of Geriatrics and Chronic Illness
• Understanding of Advanced Illness and end-of-life discussions
• Proficient computer skills, including documenting medical information with written and electronic medical records
• Demonstrated ability to develop and maintain positive customer relationships
• Proven adaptability to change

Pay

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors, including local labor markets, education, and work experience. Compensation may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
