About the Role

As a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will conduct comprehensive in-home health assessments for Medicare Advantage and other plan members. The role emphasizes patient education, addressing social determinants of health, and recognizing urgent situations for intervention.

Responsibilities

• Perform in-home assessments, including vital signs, screenings, and tests
• Document diagnoses and communicate findings to primary care providers (PCPs)
• Educate members on health management and medication compliance
• Identify social needs and provide appropriate referrals
• Recognize and respond to urgent health issues

Requirements

• Active licensure and certification (ANCC, AANP, or NCCPA)
• Unrestricted license and valid driver’s license with reliable transportation
• Ability to navigate home environments and perform physical assessments

Preferred Skills

• 1+ years of clinical experience
• Proficiency with electronic medical records (EMRs)
• Strong communication skills, particularly with older populations

Pay

Salary ranges from $109,500 to $164,000, including base pay and bonuses.
