Registered Nurse (Part Time)

About the role

Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states. We are seeking a Part Time Registered Nurse (RN) in Nashua, NH.

Responsibilities

• Provide treatment and health care procedures ordered by a physician and required by the standards of professional practice or law
• Ensure individuals attend medical appointments per their Individual Support Plan (ISP)
• Advise supervisor of all concerns and issues related to the provision of nursing services
• Create and distribute medical/health reports detailing general health, health care goals, and physician recommendations
• Conduct programmatic trainings including medication administration and delegation
• Develop and update Individual Support Plans (ISP)/Person Centered Plans (PCP) with an interdisciplinary team to include any concerns, observations, and behavioral developments
• Train employees on the medical needs of assigned individuals
• Monitor health and medical conditions of individuals and refer for medical services as needed
• Monitor the use of medications through medication administration logs
• Ensure health service supplies are available and replenished as needed
• Ensure program documentation and billable records are completed accurately and timely
• Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families
• Participate with individuals in community, social, and recreational activities
• Participate in circles of support for personal futures planning
• Follow strict OSHA procedures regarding blood borne pathogens
• Additional tasks and responsibilities may be assigned

Requirements

• Bachelor’s degree or equivalent experience in nursing field
• Minimum of one year of experience providing nursing services to individuals with disabilities
• Valid R.N. license
• Valid driver’s license with a satisfactory driving record
• Complete all state and agency required training per state guidelines
• Ability to calculate figures related to medication dosages
• Understanding and commitment to community-based support for persons with disabilities
• Proficient with Microsoft Office
• Effective communication skills with program participants, third-party agencies, staff, and members of the community

Qualifications

• Ability to work in office, residential programs, day programs, and community environments
• Exposure to changing weather conditions when working outside
• Flexibility to work evenings and weekends as needed
• Occasional overtime may be required
• Frequent lifting, stretching, and other physical exertion may be required
• May be required to lift or move 50+ pounds
• May be required to assist with wheelchair transfers of non-ambulatory individuals
• May be required to transport individuals utilizing own vehicle or company provided vehicles
• Exposure to various medical conditions, communicable diseases, blood, body fluids, or tissues
