NP or PA - Part Time - Senior Community Care - Marion and Polk Counties, OR

About the role

Optum Home & Community Care is seeking a Nurse Practitioner or Physician Assistant to join our Senior Community Care team in Marion and Clackamas County, Oregon. Responsibilities

• Conduct comprehensive assessments
• Manage patients through acute medical situations or exacerbations of chronic conditions
• Ensure accurate and complete documentation, including ICD 10 conditions
• Communicate and collaborate with the interdisciplinary care team
• Provide patients and caregivers with counseling and education
• Perform comprehensive assessments and document findings in compliance with CMS regulations
• Facilitate agreement and implementation of the patient’s plan of care
• Coordinate care as patients transition through different levels of care and care settings
• Review orders and interventions for appropriateness and response to treatment
• Identify most effective plan of care that aligns with the patient’s needs and wishes
• Function independently and responsibly with minimal need for supervision

Requirements

• Nurse Practitioner: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program, active and unrestricted license, board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in Family Nurse Practitioner, Adult Nurse Practitioner, Gerontology Nurse Practitioner, Adult-Gerontology Acute Care Nurse Practitioner, or Adult-Gerontology Primary Care Nurse Practitioner
• Physician Assistant: Graduate of an accredited Physician Assistant degree program, active and unrestricted license, board certified by the NCCPA, active DEA licensure/prescriptive authority, ability to gain a collaborative practice agreement, if applicable in your state, ability to complete physical requirements, proficient computer skills including the ability to document medical information with written and electronic medical records

Qualifications

• New graduates are encouraged to apply
• 1+ years of clinical experience in practice in senior living, geriatric medicine, or home care
• Experience in meeting the healthcare needs of patients with complex behavioral, social and/or functional needs
• Understanding of Geriatrics and Chronic Illness
• Understanding of Advanced Illness and end of life discussions
• Proficient computer skills including the ability to document medical information with written and electronic medical records

Pay

$40,000 student loan repayment or $30,000 sign-on bonus, plus $2,500 relocation assistance
