Part-Time LPN Patient Services Coordinator

About the Role

The Patient Services Coordinator, LPN/LVN, serves as a key link between clinical staff and patients, managing healthcare maintenance and treatment scheduling. They communicate complex medical information in simple terms to patients, ensuring understanding and effective conveyance of concerns to the medical team.

Responsibilities

• Manage patient schedules, including supervisory, discharge, recertification, and transfer visits.
• Reschedule missed visits and prepare weekly agent schedules.
• Assist with case conferences and handle clinical questions from field staff.
• Participate in patient visits and on-call rotations as needed.
• Maintain logs, enter notes, verify visit documentation, and run scheduling reports.
• Support patient transfers between branches and perform additional duties based on branch needs.

Requirements

• Licensed Professional Nurse or Licensed Vocational Nurse licensed in the practicing state.
• At least 1 year of home health experience.

Qualifications

• Preferred: packet review/QI experience and coding certification.
