Part Time LPN or RN - Home Care RAI Visits
About the role
Join us at BAYADA/Integrity Home Care as a valued member of our team. As a nurse with BAYADA/Integrity Home Care, you will work one on one with patients in their home and community to help them maintain their independence and enjoy life to the fullest.
Responsibilities
- Conducting annual RAI and GHE assessments with Medicaid clients receiving in-home services
- Evaluating functional abilities, health status, and care needs
- Accurately documenting findings and maintaining compliance with state requirements
- Collaborating with clients, caregivers, and interdisciplinary teams
- Supporting the reauthorization process for Medicaid-funded services
- Monitoring changes in client status and identifying potential care needs
Qualifications
- Current MO RN or LPN license in good standing
- Current CPR certification
- Strong organizational and communication skills
