About the Role
The Hospice Registered Nurse provides and documents skilled nursing care according to individualized care plans and physician’s orders, ensuring confidentiality and professional boundaries.
Responsibilities
- Deliver and coordinate quality patient care in compliance with physician directives
- Assess patient condition continuously and report changes
- Document assessments, actions, and care activities promptly
- Develop, implement, and update nursing care plans
- Take appropriate nursing actions based on assessments to achieve desired outcomes
- Recognize and respond to patient needs and risks
- Ensure compliance with policies, regulations, and standards
- Educate patients and families on disease management, self-care, and prevention
- Maintain professional competencies through ongoing education
Requirements
- Graduate from an accredited nursing program (Diploma, Associate, or Baccalaureate)
- Valid, unrestricted RN license in the state of practice
- Current CPR certification
- Valid driver’s license with insurance and acceptable driving record
