Hospice RN Case Manager Part Time

About the Role

The Hospice Registered Nurse provides and documents skilled nursing care according to care plans and physician’s orders, maintaining confidentiality and professional boundaries.

Responsibilities

• Deliver and coordinate quality patient care in compliance with physician directives.
• Assess patient conditions continuously, report changes, and update care plans accordingly.
• Document assessments, actions, and responses promptly.
• Implement nursing interventions to achieve outcomes and prevent risks.
• Ensure compliance with policies, regulations, and the Nurse Practice Act.
• Follow infection control standards and educate patients and families on disease management, self-care, and prevention.
• Maintain professional competencies through ongoing education and skill evaluations.

Requirements

• Graduate of an accredited nursing program (Diploma, Associate, or Baccalaureate).
• Valid, unrestricted RN license in the state of practice.
• Current CPR certification.
• Active driver’s license with valid automobile insurance.
