Hospice RN Case Manager - Part Time

About the role

The Registered Nurse Case Manager provides comprehensive in-home hospice care, responsible for developing and implementing individualized Plan of Care in collaboration with patients, families, and healthcare professionals.

Responsibilities

• Conducting initial evaluations
• Assessing evolving patient needs
• Documenting care
• Educating families on self-care techniques

Qualifications

• Current RN licensure in the state
• Valid driver's license
• Reliable transportation
• Flexibility to travel within a designated service area
• Availability for evening and weekend on-call rotations

Schedule

Part-time schedule with approximately 20 hours weekly.
