Responsibilities

• Implementing care plans in collaboration with licensed Physical Therapists
• Guiding therapeutic exercises and assisting with mobility and gait training
• Monitoring and documenting patient progress in EMR systems
• Collaborating with nurses and caregivers to ensure comprehensive care
• Educating patients and families to promote safe, independent living at home
