We are seeking a Licensed Practical Nurse (LPN) to play an integral role in our team, contributing to the well-being of our community and creating a home our residents are proud of. The primary purpose of this position is to provide high-quality, holistic nursing care in accordance with nursing practice standards and healthcare regulations.
Responsibilities
- Lead or direct licensed and non-professional staff in delivering direct resident care and support functions.
- Nursing Care Functions:
- Conduct daily nursing functions in accordance with current rules, regulations, and guidelines governing long-term care/assisted living communities.
- Ensure compliance with written policies and procedures, including the safety program, among assigned nursing personnel.
- Admit, transfer, and discharge residents with proper documentation of care and services provided.
- Participate in orienting new residents and family members to the facility.
- Make rounds with healthcare providers, Hospice staff, and Home Health staff as needed.
- Utilize nursing personnel qualifications according to resident care plans, service plans, and healthcare provider orders.
- Communication/Documentation Functions:
- Complete daily clinical and environmental rounds to evaluate resident care and report significant events.
- Address and resolve issues with residents and/or family members; report significant issues to the supervisor.
- Participate in facility surveys (inspections) as requested by management.
- Manage clinical programs as directed (e.g., dementia care, wound care, quality assurance, infection control, admissions, and falls prevention).
- Complete initial assessments and assist in developing interim care/service plans.
- Update, review, and transcribe care plans to identify goals, problems, approaches, and revisions based on nursing needs and healthcare provider orders.
- Assist with service plans as directed by the Director of Nursing.
- Communicate with healthcare providers to review treatment plans, complete orders, and document progress notes.
- Review resident medical records for treatments, medication orders, diets, and labs to ensure implementation and follow-up.
- Requisition and arrange diagnostic and therapeutic services as ordered by healthcare providers.
- Communicate department policies and procedures to personnel, residents, and visitors.
- Provide electronic or written (24-hour report) and oral reports on resident condition changes and shift activities to nursing management.
- Complete and submit accident/incident reports involving residents to the Director of Nursing.
- Document all required medical records accurately and according to policy.
- Provide resident and family information for care conferences or other meetings.
- Report discrepancies in physician orders, diet changes, or charting errors to the supervisor.
- Review interdisciplinary team progress notes to ensure care/service plans are followed; report concerns to the supervisor.
- Inform nursing personnel of new admissions and resident care needs; ensure rooms are prepared and greet new residents.
- Notify healthcare practitioners and family members of changes in resident condition.
- Assist in arranging transportation and third-party services for residents.
- Medication Administration Functions:
- Prepare and administer medications as ordered by healthcare providers.
- Communicate with the pharmacy to ensure resident medications are available.
- Maintain an adequate supply of floor stock medications, supplies, and equipment; report needs to the supervisor.
- Review medication administration records for completeness and accuracy; report concerns to the supervisor.
- Dispose of drugs and narcotics according to established procedures.
- Personnel Functions:
- Supervise the nursing unit to ensure personnel perform work assignments according to acceptable nursing standards.
- Work with the Director of Nursing to resolve staff performance issues, including those that may lead to termination.
- Report complaints and grievances to management and collaborate on action plans.
- Safety and Sanitation Functions:
- Ensure protective equipment is available for staff.
- Maintain clean and sanitary nursing service work areas, resident rooms, and treatment areas.
- Recommend equipment and supply needs to the Director of Nursing.
- Maintain stock levels of medications, medical supplies, and equipment to meet resident needs.
- Perform other job-related duties as assigned.
Requirements
- Meet all applicable state and federal requirements for the position.
- Possess a current, unencumbered, active LPN/LVN license in the applicable state, in good standing throughout employment with no disciplinary action.
- One year of experience in nursing within a home care, assisted living, or long-term care facility is preferred.
- Knowledge and Critical Skills:
- Ability to make independent decisions and follow instructions.
- Tactful communication with personnel, residents, family members, visitors, government agencies, and the public.
- Capability to work with ill, disabled, elderly, and emotionally upset individuals.
- Effective communication with supervisors, team members, residents, and families.
- Supervisory Responsibility:
Ensure resident care is provided according to standards of practice and facility policies by CNAs/Caregivers, though CNAs/Caregivers do not directly report to this position.
- Working Conditions and Physical Demands:
- Potential and/or direct exposure to blood, body fluids, infectious diseases, air contaminants, and hazardous chemicals.
- Physical Requirements:
- Walk long distances and stand for extended periods.
- Sustain repetitive movements (e.g., CPR compressions, bagging).
- Maintain physical tolerance to stand, walk, or work on your feet for 8-12 hours with minimal breaks.
- Lift up to fifty pounds at times.
- Push and pull up to fifty pounds at times.
- Cognitive Requirements:
- Execute tasks independently.
- Express yourself clearly and effectively.
- General computer literacy.
- Environmental Requirements:
Exposure to ambient room temperatures and lighting.
Schedule
6:30 AM – 3:30 PM, every other weekend.
Pay
$30–$35 per hour.
