About the Role
The primary purpose of this position is to provide each assigned resident with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as directed by supervisors.
Responsibilities
- Use the wristband or photo card file to identify residents before administering treatments, serving meals, etc.
- Use only authorized abbreviations established by the facility when recording information.
- Report all changes in the resident’s condition to the Nurse Supervisor/Charge Nurse as soon as practical.
- Record all entries on flow sheets, notes, charts, etc., in an informative, descriptive manner.
- Report all accidents and incidents observed on the shift they occur.
- Agree not to disclose resident’s protected health information and promptly report suspected or known violations.
- Report any known or suspected unauthorized attempt to access the facility’s information system.
- Perform only those nursing care procedures for which you have been trained.
- Ensure the resident’s room is ready for receiving the resident (e.g., bed made, name tags up, admission kit available).
- Greet residents and escort them to their room; introduce them to roommates and personnel as appropriate.
- Make residents comfortable, inventory and mark personal possessions, and store resident’s clothing.
- Assist residents with packing personal possessions during transfers or discharges.
- Transport residents to new rooms or receiving areas; assist with loading/unloading residents to/from vehicles.
- Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment.
- Meet with nursing personnel regularly to identify and correct problem areas or improve services.
- Report all complaints and grievances made by residents.
- Participate in and receive nursing reports as instructed.
- Follow established policies concerning exposure to blood/body fluids.
- Make beds, measure and record temperatures, pulses, respirations (TPRs), and weigh residents as instructed.
- Ensure residents who cannot call for help are checked frequently; answer resident calls promptly.
- Check residents routinely to ensure personal care needs are met.
- Assist residents with food arrangements (e.g., informing visually impaired residents of food location, temperature).
- Record resident’s food/fluid intake and report changes in eating habits.
- Keep residents’ water pitchers clean, filled, and within easy reach.
- Perform after-meal care (e.g., remove trays, clean residents’ hands, face, clothing).
- Perform all assigned tasks in accordance with established policies and procedures.
- Follow work assignments and schedules to complete tasks.
- Cooperate with inter-departmental personnel to maintain nursing services.
- Notify the facility when late or absent from work.
- Report occupational exposures to blood, body fluids, infectious materials, or hazardous chemicals.
- Assist residents with daily functions (e.g., dental/mouth care, bathing, dressing, combing hair).
- Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
- Assist residents in preparing for medical tests (e.g., lab work, x-ray, therapy, dental).
- Assist with lifting, turning, moving, positioning, and transporting residents.
- Perform restorative and rehabilitative procedures as instructed.
- Prepare residents for physical examinations.
- Check residents routinely to ensure personal care aligns with their wishes.
- Observe and report pressure areas, skin breakdowns, or injuries of unknown source.
- Observe disoriented and comatose residents; record and report data as instructed.
- Perform special treatments as instructed.
- Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving or missing from the facility.
- Use only equipment you have been trained to use; report defective equipment.
- Maintain confidentiality of all resident care information, including protected health information.
- Report allegations of resident abuse and/or misappropriation of resident property.
- Honor the resident’s refusal of treatment and report such requests to your supervisor.
Requirements
- Must possess, at minimum, a 10th-grade education.
- Ability to read, analyze, and interpret professional journals, technical procedures, or governmental regulations.
- Ability to write reports, business correspondence, and procedure manuals.
- Ability to effectively present information and respond to questions from managers and employees.
- Ability to apply mathematical concepts such as fractions, percentages, ratios, and proportions to practical situations.
- Ability to solve practical problems and deal with variables in situations with limited standardization.
- Ability to interpret instructions furnished in written, oral, diagram, or schedule form.
- Must be a licensed Certified Nursing Assistant in accordance with state laws.
- CPR certification preferred.
Physical Demands
- Must occasionally lift and/or move up to 25 pounds.
- Prolonged use of a desktop or laptop computer.
- Regularly required to sit, stand, walk, talk, read, or hear.
- Frequent use of office equipment (e.g., copier, scanner, fax, telephone, calculator).
- May be necessary to assist in evacuating residents during emergencies.

- The noise level in the work environment is usually low to moderate.
