About the Role

The primary purpose of this position is to provide each assigned resident with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as directed by supervisors.

Responsibilities

  • Use the wristband or photo card file to identify residents before administering treatments, serving meals, etc.
  • Use only authorized abbreviations established by the facility when recording information.
  • Report all changes in the resident’s condition to the Nurse Supervisor/Charge Nurse as soon as practical.
  • Record all entries on flow sheets, notes, charts, etc., in an informative, descriptive manner.
  • Report all accidents and incidents observed on the shift they occur.
  • Agree not to disclose resident’s protected health information and promptly report suspected or known violations.
  • Report any known or suspected unauthorized attempt to access the facility’s information system.
  • Perform only those nursing care procedures for which you have been trained.
  • Prepare the resident’s room for admission (e.g., bed made, name tags up, admission kit available).
  • Greet residents, escort them to their room, and introduce them to roommates and personnel as appropriate.
  • Make residents comfortable, inventory and mark personal possessions, and store clothing.
  • Assist residents with packing personal possessions for room transfers or discharges.
  • Transport residents to new rooms or the receiving area, and assist with loading/unloading residents to/from vehicles.
  • Maintain an atmosphere of warmth, personal interest, and a calm environment throughout the unit.
  • Meet with nursing personnel regularly to identify and correct problem areas or improve services.
  • Report all resident complaints and grievances.
  • Participate in and receive nursing reports as instructed.
  • Follow established policies concerning exposure to blood/body fluids.
  • Make beds, measure and record temperatures, pulses, respirations (TPRs), and weigh residents as instructed.
  • Check on residents unable to call for help frequently and answer resident calls promptly.
  • Assist residents with food arrangements, record food/fluid intake, and report changes in eating habits.
  • Keep residents’ water pitchers clean, filled, and within easy reach.
  • Perform after-meal care (e.g., remove trays, clean residents’ hands, face, clothing).
  • Perform all assigned tasks in accordance with established policies and procedures.
  • Follow work assignments and schedules to complete assigned tasks.
  • Cooperate with inter-departmental personnel to maintain nursing services.
  • Notify the facility when late or absent from work.
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
  • Assist residents with daily functions (e.g., dental and mouth care, bathing, dressing, combing hair).
  • Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
  • Assist residents in preparing for medical tests (e.g., lab work, x-ray, therapy, dental).
  • Assist with lifting, turning, moving, positioning, and transporting residents.
  • Perform restorative and rehabilitative procedures as instructed.
  • Prepare residents for physical examinations.
  • Check residents routinely to ensure personal care needs are met.
  • Observe and report pressure areas, skin breakdowns, and injuries of unknown source.
  • Observe disoriented and comatose residents and record/report data as instructed.
  • Perform special treatments as instructed.
  • Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving or missing from the facility.
  • Use only equipment you have been trained to use and report defective equipment.
  • Maintain confidentiality of all resident care information, including protected health information.
  • Report allegations of resident abuse and/or misappropriation of resident property.
  • Honor the resident’s refusal of treatment and report such requests to your supervisor.

Requirements

  • Must possess, as a minimum, a 10th grade education.
  • Ability to read, analyze, and interpret professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from managers and employees.
  • Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
  • Ability to solve practical problems and deal with variables in situations with limited standardization.
  • Ability to interpret instructions furnished in written, oral, diagram, or schedule form.
  • Must be a licensed Certified Nursing Assistant in accordance with state laws.
  • CPR certification preferred.

Physical Demands

  • Must occasionally lift and/or move up to 25 pounds.
  • Prolonged use of a desktop or laptop computer.
  • Regularly required to sit, stand, walk, talk, read, or hear.
  • Frequent use of office-related equipment (e.g., copier, scanner, fax, telephone, calculator).
  • May be necessary to assist in the evacuation of residents during emergency situations.

Work Environment

  • The noise level in the work environment is usually low to moderate.

Pay

$24.23 - $29.23 per hour.
