Responsibilities

• Provides dementia care including adapted ADLs, meaningful engagement, and communication to residents following a person-centered individual service plan.
• Treats residents with respect and dignity, recognizes individual needs, and encourages independence.
• Fosters a purposeful and engaging culture throughout the community.
• Assists residents and provides input regarding the care plan and daily living activities, including bathing, dressing, grooming, toileting, transferring, and participation in programs and meals according to the individual service plan and interventions.
• Partners with and encourages residents to be as independent as possible.
• Participates in team behavioral expression meetings or discussions with community leadership.
• Knows each resident’s Life Story, preferences, historical routines, and interests.
• Serves meals to residents in the dining room; may assist in preparing meals following preplanned dementia-friendly menus.
• Encourages residents, using prompting and cueing skills, to eat a proper diet and take adequate fluids while respecting the resident’s right to free choice; records and reports changes in eating habits.
• Initiates and/or leads at least one Daily Path engagement program tailored to residents’ Life Story, Solace program, or calendar; encourages socialization and participation in planned or spontaneous programs.
• Maintains residents’ records daily, documenting engagement interests, incidents, observations, changes in physical condition or behavior, visitors, and medication distribution (as applicable).
• Observes and promptly reports any changes in residents’ health or emotional condition to the supervisor.
• If permitted by state regulations, assists residents with medication as defined in medication procedures, follows physician orders, and adheres to state laws and company policies.
• Maintains a clean, safe, and orderly environment; performs general housekeeping including laundry, bedrooms, dining areas, living spaces, and bathrooms.
• Follows proper procedures in emergency situations and responds promptly to resident requests, including emergency pull cords, telephone calls, and requests from family and friends.
• Monitors resident locations and notifies the supervisor immediately if a resident cannot be located.
• Encourages teamwork through cooperative interactions with co-workers.

Requirements

• High school diploma or GED; three to six months of related experience and/or training in direct service with older adults living with dementia, or equivalent combination of education and experience.
• Knowledge of dementia, particularly Alzheimer’s type.
• Refer to specific state regulations for qualifications required to assist with medications and direct care duties.
• In accordance with state law, may need to possess and maintain current state certification.

Skills

• Ability to read, comprehend, and follow simple instructions, short correspondence, and memos.
• Ability to write simple correspondence.
• Ability to communicate effectively in English with residents, family members, and associates.
• Ability to apply common-sense understanding to carry out instructions furnished in written or oral form.
• Uses good judgment in emergency situations and seeks assistance when needed.

Base pay in range will be determined by applicant’s skills and experience.
