About the role
The Mobile Integrated Health Medic for Population Health Management provides comprehensive, safe, and efficient in-home care under the direction of a physician. This position enhances a patient's experience by offering an alternative to emergency department visits or hospitalizations.
Responsibilities
- Perform comprehensive assessments, diagnostic testing, and medical treatments in patients' homes or current residences.
- Work collaboratively with the care team to provide holistic patient care.
- Ensure compliance with all regulatory standards and protocols.
- Document patient interactions and treatment plans accurately and timely.
- Communicate effectively with patients, families, and other healthcare providers.
Requirements
- Three to five (3-5) years of clinical experience.
- Community Paramedic experience desirable.
- Strong clinical skills with knowledge of emergency medicine and chronic disease management.
- Extensive knowledge of pharmacology, anatomy, and pathophysiology.
- Extensive understanding of community resources and diverse populations.
- Excellent customer service and interpersonal skills.
- Strong problem-solving, analytical, and decision-making skills.
- Strong organizational, planning, and implementation skills.
- Able to balance multiple tasks simultaneously.
- Creative and resourceful with critical thinking skills.
- Excellent written and verbal communication and documentation skills.
- Ability to work independently with minimal supervision.
Qualifications
- Licensure as a Paramedic with a valid State of Michigan license.
- American Heart Association - Basic Life Support (BLS) certification.
- American Heart Association - Advanced Cardiovascular Life Support (ACLS) certification.
- Valid State of Michigan Driver's License.
