Key Responsibilities Include Delivering specialized primary care, including prescribing and on-call duties, in collaboration with a supervising physician Managing care for patients with complex conditions through comprehensive assessments and interdisciplinary team coordination Conducting face-to-face patient visits with proper documentation Providing clinical education and acting as a resource for staff and team initiatives Supporting performance improvement and contributing to clinical excellence Qualifications Required Unrestricted state license and certification as a Clinical Nurse Specialist or Nurse Practitioner National certification from ANCC or AANP Possession of NPI, DEA with prescribing privileges, PECOS, CAQH, Medicare, and Medicaid numbers Current CPR certification, valid driver’s license, and reliable transportation At least one year of NP experience in chronic disease, family, or primary care Compensation and benefits: Competitive salary range, including base pay and bonuses, plus comprehensive benefits, incentives, 401k, and stock options.
