Palliative Care Nurse Practitioner

About the role

This position offers comprehensive, patient-centered care to older adults in palliative or hospice settings. The ideal candidate will have an active Missouri NP license (RN + NP), certification as an Adult or Geriatric Nurse Practitioner, experience in palliative care, hospice, geriatrics, or complex care management, and strong clinical judgment.

Responsibilities

• Provide comprehensive, patient-centered care to older adults in palliative or hospice settings
• Conduct assessments, diagnose conditions, and develop individualized care plans
• Collaborate with physicians and interdisciplinary teams to improve patient outcomes
• Support patients and families through serious illness, focusing on comfort and quality of life
• Prescribe and manage treatments in accordance with state guidelines and collaborative agreements
• Educate patients, caregivers, and staff on care plans and disease progression
• Coordinate care across settings and connect patients to appropriate resources

Requirements

• Active Missouri NP license (RN + NP)
• Certification as an Adult or Geriatric Nurse Practitioner
• Experience in palliative care, hospice, geriatrics, or complex care management
• Palliative Care certification (or willingness to obtain)
