Nurse Practitioner
About the role
Matter Health is seeking an Advanced Practice Provider/Nurse Practitioner to support older adults in Springfield, MA.
This is a fully community-based position with no traditional clinic home base. APPs work with a high degree of clinical autonomy while collaborating closely with an interdisciplinary care team and a dedicated Medical Director for support in complex cases.
Key Responsibilities
- Provide ongoing primary and preventive care to older adult patient populations
- Manage and grow a dedicated patient panel within assigned communities
- Deliver relationship-driven, patient-centered care in community settings
- Collaborate with interdisciplinary clinical and operational teams
- Support initiatives in preventative care, chronic disease management, and care coordination
- Utilize a combination of telehealth and in-person care delivery models to improve accessibility
- Maintain accurate, timely EHR documentation and ensure coding specificity
Qualifications
- Active, unrestricted Nurse Practitioner or Physician Associate license
- Board Certification: NP (AANPCB or ANCC — family, adult, geriatric, or adult-gerontology) PA (NCCPA)
- Active DEA license and controlled substance registration, as required
- Basic Life Support (BLS) certification
- Valid driver’s license and reliable transportation (car stipend provided)
- Experience with EHR documentation (athenahealth preferred)
- Commitment to equitable, patient-centered care
- Strongly Preferred: Background in family medicine, internal medicine, adult medicine, or geriatrics
- Experience in value-based care settings
- Experience providing care in community-based, in-home, or non-traditional healthcare environments
- Familiarity with MEAT documentation and chronic condition coding
- Telehealth experience
