Responsibilities
- Provide direct patient and family care through palliative care consultations for patients with serious illnesses not eligible for hospice.
- Apply evidence-based palliative medicine practices ethically and effectively, considering physical, emotional, spiritual, psycho/social, ethical, educational, and safety criteria.
- Conduct comprehensive assessments using palliative care domains, interpret findings, perform medication reconciliation, diagnose, develop, and evaluate care plans with therapeutic interventions.
- Gather pertinent clinical information from multiple sources, including medical records and family input.
- Incorporate clinical expertise and palliative care knowledge to formulate effective care strategies.
- Encourage ACHPN certification after gaining experience.
Requirements
- RN/APRN licensure with prescriptive authority in the practice state.
- Minimum of 5+ years of clinical nursing experience.
- Current BCLS and CPR certifications.
- Valid driver’s license and reliable transportation.
Preferred Qualifications
- Advanced practice palliative care certification (ACHPN).
- 1+ years as APRN in Hospice, Palliative Care, or Oncology.
