Responsibilities
- Provide primary and urgent care services to nursing center residents in collaboration with the other medical providers.
- Collaborate with Center Leadership to improve health outcomes and provide high-quality care.
- Offer palliative care services to nursing center residents with serious illness, focusing on pain and symptom management, psychosocial support, and advance care planning.
- Consult with the primary care team to provide recommendations for the plan of care, incorporating patient goals and preferences.
- Develop and implement advanced care planning for all residents.
- Follow evidence-based practice guidelines, assess health status, diagnose, interpret labs and diagnostics, develop and implement treatment plans, and evaluate patient status.
- Document health care services according to established policies and procedures and relevant state and federal regulatory requirements.
- Provide health education to patients and families.
- Demonstrate continued professional competence through ongoing learning and participation in clinical initiatives.
- Support the development of nursing staff and participate in center-based quality initiatives.
- Adhere to coding and billing compliance program and comply with AlignMed Partners' Time Off and On Call policies.
Qualifications
- Graduate from an accredited School of Nursing program and licensed as a registered nurse.
- Certified as a Nurse Practitioner from an accredited Nurse Practitioner Program with a Master’s Degree in Nursing and a concentration in Older Adult or Family Nurse Practitioner (FNP).
- A minimum of two years experience working with the adult or geriatric population, with preference given to those with experience in states where practice allows.
- In states where practice allows, an unrestricted professional license and DEA for each state in which they practice.
Pay
USD $120,000.00 - USD $130,000.00 /Yr.
Schedule
Full-time (40 hours per week)
