Palliative Care Coordinator

About the Role

The Palliative Care Coordinator is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. They uphold the Code of Ethical Conduct and promote positive working relationships within the company, among all departments, and with external stakeholders. The role involves managing the patient chart from initial referral through discharge, ensuring compliance with Medicare/Medicaid rules and regulations, facilitating patient care flow, scheduling, phone triage, billing, and maintaining patient metrics.

Responsibilities

• Supports agency and corporate policies, goals, and objectives.
• Attends and participates in staff meetings, educational programs, and community events as requested.
• Communicates identified needs and potential solutions to supervisor.
• Establishes and maintains patient charts, ensuring all referral documentation is signed and dated.
• Submits requests for medical records and hospital inpatient charts.
• Files documents accurately in patient charts and thins charts as necessary.
• Schedules new and follow-up Community Palliative Care visits and confirms appointments the day before.
• Answers incoming telephone calls, takes accurate messages, and transfers calls appropriately.
• Rotates deceased/discharged patient charts from active status and establishes files for them.
• Performs billing tasks as outlined in the Palliative Care Guideline.
• Coordinates communications with members of the Palliative Care service, patients, families, and the community.
• Transfers phone system to secondary system as appropriate.
• Faxes documentation of Palliative Care encounters to attending physician/Primary Care Physician.
• Performs other duties as assigned.

Requirements

• High school diploma or GED required.
• Two (2) to three (3) years of related experience highly preferred.
• Experience as a Licensed Practical/Vocational Nurse preferred.

Skills

• Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
• Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
• Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.

Physical Demands and Work Environment

The demands of this role necessitate regular standing, walking, and manual dexterity, along with the ability to lift and move objects up to 25 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. Adaptations can be made to accommodate team members with disabilities.

Pay

Estimated salary range: $15.45 - $24.26 per hour. Actual salary will vary by geographic location and experience.
