Palliative Care Community Based Advanced Practice Opportunity
Position Highlights
Join our vibrant division of Palliative Care that includes 6 Advanced Practice Providers (APPs), 6 physicians, 2 social workers, spiritual support and a highly supportive clinical staff dedicated to providing evidence-based and patient-centered care.
Our community palliative care program focuses on providing patient-centered care in a supportive environment to patients and families facing advanced illness and end-of-life challenges.
The position involves collaborating closely with an interdisciplinary team consisting of Social Work, Nursing, Spiritual Care Services, Nurse Practitioners and Physicians.
Candidates will participate in interdisciplinary team meetings that foster collaboration, professional development, and continuous growth.
This is a .8 FTE community-based position. Schedule offers flexibility providing great work/life balance.
Mileage reimbursement and laptop provided. Regions are assigned based on geographic location.
Qualifications
BC Nurse Practitioner or Physician Assistant with clinical experience in home care, palliative care, or geriatrics settings.
Pay
Palliative Care AP: $129,780 - $164,400 for 1.0 FTE, $103,824 – 131,520 for .8 FTE
