Advanced Practice Provider Palliative Care

About the role

As an Advanced Practice Provider (APP), you will provide advanced clinical care to assigned patient populations while serving as a key member of an interdisciplinary team.

Responsibilities

• Assessing, collecting, and analyzing patient health data
• Developing, implementing, and evaluating individualized plans of care
• Coinciding comprehensive patient care and case management services
• Providing expert clinical consultation
• Utilizing prescriptive authority, procedures, and treatments in accordance with state and federal regulations
• Manning high-risk patient populations
• Supporting quality improvement and research initiatives
• Promoting positive patient outcomes and enhancing care delivery systems

Requirements

• Applicable Experience: Less than 1 year
• Advanced Practice Provider (MO) - Various
• Advanced Practice Provider Cert(MO/KS) - Various
• Advanced Practice Provider - Various
• Master's Degree
