Shift: Monday to Friday, 8:00 AM to 4:30 PM • Full time • Location: Mercy LIFE Valley View, Elwyn, PA

About the Role

This position will plan, organize, and implement social services for PACE Organization (PO) participants and families including case management and participant advocacy. Responsible for assessment, treatment, teaching, and counseling participants, caregivers, or other appropriate representatives. Mercy LIFE Valley View provides both housing and the Mercy LIFE program for senior adults, serving a community specifically comprised of deaf and/or blind participants over the age of 55. Candidates must be willing to learn American Sign Language (ASL); interpreters and/or other tools will be provided during training. Hours will be worked exclusively at the facility site.

Responsibilities

• Assess participants and coordinate care plans with the Interdisciplinary Team (IDT).
• Conduct intake meetings, provide counseling and support.
• Act as liaison with agencies and provide resource referrals.
• Coordinate discharge planning and transitional care.
• Provide individual and family counseling.
• Act as liaison between the participant and other agencies and facilitate discharge placement planning.

Requirements

• Master’s degree in social work.
• Two (2) years of experience.
• Current social work license preferred.
• One (1) year working with the frail or elderly population strongly preferred.
• Experience working on a multidisciplinary team in a medical/community-based setting preferred.
• Knowledge of Medicare, Medicaid, and other payment systems.
• Valid driver's license, insurance, and use of personal vehicle.
• Superior written and verbal communication skills.
• Proficiency with MS Office.
• Ability to manage conflict well, high decision quality, and flexibility to adapt to ongoing change.
