About the role
The Center Manager oversees the overall operations of the PACE Center. This role supervises receptionists, center caregivers, escorts, and dietary aides ensuring scheduling, training, performance management, and quality standards.
Responsibilities
- Provide direct supervision, training, and evaluation to receptionists, center caregivers, escorts, and dietary aides.
- Ensure effective hiring, training, scheduling, coverage, and staffing for the PACE Center.
- Develop policies and procedures for all center operations.
- Facilitate and lead morning huddle meetings and interdisciplinary team (IDT) meetings.
- Cookordinate and schedule all center activities and events including interdisciplinary conference and daily huddle meetings, family conferences, and initial assessments and reassessments.
- Work closely with PCPs and other disciplines to develop and implement individualized care plans.
- Monitor scheduling, center services encounters, assessment, and care-plan metrics and identify opportunities to improve overall participant experience.
- Resolve and communicate with participants and families on grievances related to center-specific issues.
- Serve as the on-site de-escalation lead for participants exhibiting challenging or disruptive behavior, intervening directly and coordinating staff response to ensure participant and staff safety.
- Cookordinate activities and resolves issues with transportation vendors.
- Track and address non-medical after hours calls.
- Lead the training, coordination, and resolution of all site and facility related issues.
- Lead meal vendor contracting and management for the site.
- Implement strategies to meet or exceed participant satisfaction benchmarks at national and state levels for center aides, meals, and general service delivery.
- Lead and coordinate QI initiatives as needed related to the center.
- Prepare for and participate in CMS and DHCS audits, and implement corrective actions as needed.
- Ensure compliance with all PACE regulatory requirements and internal policies.
- Oversee accurate and cohesive documentation in participant medical records.
- Oversee the quality of care and safety of PACE participants, at the PACE center.
- Utilize Epic appropriately and efficiently and serve as content and workflow expert for resolving issues and proposing solutions in Epic.
- Perform other job duties as required by manager/supervisor.
Qualifications
- Bachelor’s degree in a health-related field required, Master’s preferred.
- At least 3 years of experience in a supervisory capacity within a healthcare setting.
- Minimum one year of experience working with a frail and/or older adult population preferred.
- Minimum one year of experience working with older adults and their families in a healthcare or community agency setting is preferred.
- Must be available to work any day of the week, Monday through Sunday, depending on need.
- Candidate must be comfortable working in a dynamic environment with direct care staff and administration staff.
- Excel in organizational and supervisory skills, with an ability to effectively handle difficult situations.
- Experience in successfully managing and completing projects is required.
- Experience with senior care clinical programs similar to PACE preferred.
- Experience working with clinical charts and records.
- Knowledgeable of the physical, mental and social needs of the elderly and be a professional in one of the following: Nursing, Social Work, Psychology, Recreation, Occupational Therapy, Physical Therapy, Speech Therapy, Dietetics, Gerontology.
- Current BLS and First Aid required.
- Language: Must be able to fluently speak, read and write English. Fluency in other languages are an asset.
