About the role
The Nurse Practitioner/Physician Assistant will evaluate, diagnose and treat patients following written guidelines approved by the Department of Medicine. They will report to the Hospitalist Program leadership team, including the Lead Advanced Practice Clinician and Program Director.
Responsibilities
- Evaluates and diagnoses patients, performing comprehensive physical exams and ordering necessary diagnostic tests and procedures.
- Manages chronic medical conditions and develops continuous care plans for hospitalized inpatients.
- Prescribes medications and writes orders in accordance with current practice guidelines.
- Maintains communication with referring physicians and collaborates with the patient care team to develop and facilitate care plans.
- Prepares patients for discharge, including teaching, arranging continuing care and rehabilitation, and confirming follow-up appointments.
- Prepares formal discharge summaries and collaborates with nursing and care progression teams.
- Manages patients to prevent complications and provides urgent evaluations for changes in clinical status.
- Possesses the maturity, self-confidence, and ability to follow complex patient management plans as directed by the attending physician.
- Ensures compliance with state and federal controlled substances regulations.
- Manages time effectively and organizes priorities, handling multiple tasks and deadlines.
- Strives to improve quality of care and works efficiently in all aspects of the position.
Requirements
- Licensed independent practitioner credentialed through the Medical Staff Services Office.
- Completion of Advanced Physician Assistant or Nurse Practitioner training program from an accredited institution.
- Demonstrated proficiency in delivering medical care to adult patients.
- Successful completion of the SSH Credentialing process.
- ACLS Certification from the American Heart Association.
- Minimum 2 years of acute medicine experience, preferred.
Qualifications
- Advanced Physician Assistant or Nurse Practitioner degree from an accredited institution.
- Current licensure and certification as a Nurse Practitioner or Physician Assistant.
- Experience with hospital medicine and emergency care.
- Ability to manage complex patient cases and maintain confidentiality.
- Excellent communication and collaboration skills.
- Strong organizational and time management abilities.

$123,947.20 - $224,122.21
Schedule
Full time - Evening/Swing shifts. Weekend requirement.
