Outreach Worker II (BHT)

About the role

This role involves working as a member of both the Certified Community Behavioral Health Clinic (CCBHC) team and the patients' treatment team. The Outreach Worker II (BHT) promotes positive behavioral and physical change for patients through clinical oversight of patient care and coordinating all covered services.

Responsibilities

• Provides and coordinates all covered services, including case management, outreach, crisis intervention, counseling, and the CCBHC nine core services.
• Identifies, locates, and engages individuals in treatment and integrated health services.
• Follows up with patients who have disengaged from services or do not show for their intake.

Requirements

• Bachelor's or Master’s degree, no experience necessary
• Bachelor’s degree in a non-behavioral health related field plus one (1) year experience providing behavioral health services
• Associate degree plus two (2) years behavioral health work experience
• High School Diploma or GED plus four (4) years of behavioral health work experience

Additional Requirements

• Must be 21 years of age (automobile insurance requirement)
• Valid AZ driver’s license with a clean or acceptable driving record
• Pass a pre-employment drug screen and background check
• Level 1 Fingerprint Clearance Card
• This is a Safety Sensitive Position
