Responsibilities
- Adhere to identified Street Outreach Safety Protocols in conducting street outreach activities.
- Organize and conduct community health fairs and facilitate educational workshops and presentations throughout the community.
- Interview individuals and document demographic, substance use, and health information as required by program protocols.
- Link individuals to substance treatment and primary health care.
- Follow up with participants who have been screened and are in need of linkage to primary or specialized health care or substance use treatment services.
- Make and track referrals for identified program participants within the PHMC Client Registry System.
- Travel throughout the city of Philadelphia.
- Meet or exceed specified outreach productivity targets as specified by program funders and/or PHMC program managers.
- Document all required program and participant demographic information into the PHMC Client Registry System within 72 hours of outreach or contact.
- May be required to work some evenings and/or weekends to support program activities.
Qualifications
- A high school diploma or GED is required.
- Prior experience in public health or social service-related work is preferred.
- Experience working with diverse populations, low-income individuals, and individuals from different religious backgrounds is preferred.
- Preferred: An associate degree.
- Preferred: Bilingual skills.

- Demonstrated ability to work with substance-involved individuals.
- Knowledge of substance use and abuse.
- Knowledge of viral Hepatitis and HIV/AIDS.
- Knowledge of public health, social services, resources, and agencies in the community.
- Knowledge of Hepatitis education, screening, and group facilitation.
- Familiarity with the project’s target area and population.
