Outreach Specialist
About the role
The HEART of Communities (HEART) Outreach Specialist serves as the first point of contact for prospective program participants. This role engages eligible patients, provides program information, supports enrollment, and maintains accurate documentation to ensure effective coordination with Community Wellness Advocates (CWAs) and clinical teams. In partnership with the Operations Manager, the Outreach Specialist supports the development and improvement of outreach workflows, enrollment goals, and special projects that advance HEART’s growth and strategic objectives.

Responsibilities
- Conduct outreach calls and chart review for new eligible HEART patients.
- Schedule intake appointments for patients who agree to participate, and assist with rescheduling for patients who no-show or cancel initial program appointment.
- Document baseline clinical data in Epic systems and maintain Epic program database for new patients.
- Assist with outreach data management, development of efficient workflows, and documenting outreach workflows and SOPs.
- Assist the HEART program with special projects as assigned by the Operations Manager.
Requirements
- High school diploma and a minimum of 1 year of customer service experience; or equivalent combination of education and experience.
Skills
- Strong oral and written communication, interpersonal, and organizational skills.
- Proficiency with software applications, including Microsoft Office and electronic health record systems (such as Epic).
Pay
Compensation range: $18.27–$25.72 per hour. This range offers an estimate based on the minimum job qualifications; base pay is determined comprehensively considering education, experience, skills, certifications/licensures, business/organizational needs, internal equity, and market competitiveness. Range is based on Boston-area data and subject to modification based on geographic location.
