Job Details
The Outreach Coordinator PFK has the following essential job functions:
- Screens for healthcare needs and connects to related programs and resources.
- Interacts daily primarily via telephone and electronic communication.
- Initiates telephone contact and completes a health risk assessment.
- Escalates complex or urgent needs to a supervisor.
- Connects patients to the right program once needs are identified.
- Develops individualized self-management skills to patients and families through education and resource provision.
- Provides organization of electronic communication related to survey results of the health risk assessment.
- Documents within the electronic medical record.
- Participates in orientation and continuing education of staff and students as appropriate.
Department Specific Job Details
- Full time, 40 hours per week.
- Monday - Friday, day shift.
- Supports Health Risk Assessment (HRA) screening program with Partners for Kids.
Education
A high school diploma is required.
Experience
Background in health care, public health, or related clinical field preferred.
Skills/Competencies
Excellent customer service skills with the ability to communicate effectively through multiple mechanisms including phone and writing are required.
