Outreach Coordinator - Partners for Kids

Job Details

The Outreach Coordinator PFK has the following essential job functions:

• Screens for healthcare needs and connects to related programs and resources.
• Interacts daily primarily via telephone and electronic communication.
• Initiates telephone contact and completes a health risk assessment.
• Escalates complex or urgent needs to a supervisor.
• Connects patients to the right program once needs are identified.
• Develops individualized self-management skills to patients and families through education and resource provision.
• Provides organization of electronic communication related to survey results of the health risk assessment.
• Documents within the electronic medical record.
• Participates in orientation and continuing education of staff and students as appropriate.

Department Specific Job Details

• Full time, 40 hours per week.
• Monday - Friday, day shift.
• Supports Health Risk Assessment (HRA) screening program with Partners for Kids.

Education

A high school diploma is required.

Experience

Background in health care, public health, or related clinical field preferred.

Skills/Competencies

Excellent customer service skills with the ability to communicate effectively through multiple mechanisms including phone and writing are required.
