Outreach Case Manager for Veterans

About the role

The Field Veteran Community Health Worker (FVCHW) acts as a vital link for homeless veterans, connecting them to essential services such as housing, healthcare, and mental health support.

Responsibilities

• Conduct street outreach in various locations within the tri-county area
• Build trust with homeless veterans
• Identify unsheltered veterans
• Complete engagement forms to assess needs
• Refer clients to appropriate organizations
• Follow up on referrals
• Provide case management: assist veterans with appointments, advocate with service providers, support goals for housing or employment
• Maintain accurate documentation in HMIS and internal systems with timely updates and reporting

Qualifications

• Bachelor's in Social Work or equivalent
• Certification or willingness to obtain Community Health Worker certification
• Excellent communication skills
• Valid driver’s license with personal transportation
• Preferred experience: working with veterans or homelessness, knowledge of VA benefits, cultural competency

Requirements

• Physical requirements: walking, standing, lifting up to 50 pounds, working outdoors in various weather conditions
